Commercial insurance clients of Northern Advantage Insurance Leaders may use the form below to request a Certificate of Insurance (COI). Please complete all the fields of information so that we may provide the most accurate COI possible. Please email a copy of the COI request that you received from the Certificate Holder IF they are requiring Additional Insured and/or Waiver of Subrogation, this can be sent to office@northernadvantageinsurance.com.
